Macomb County and its partners on the Healthy Macomb Advisory Committee are working to include feedback from Macomb County residents and all those who work or play in Macomb County.  Healthy Macomb is launching its Community Survey and every voice counts.  Please take less than 10 minutes of your day to share what health concerns and community priorities are important to you!  Healthy Macomb Survey

In 2015, the Macomb County Health Department began the process of conducting our Community Health Assessment and published a Community Health Improvement Plan in 2017. In 2020, the Macomb County Health Department initiated a second Community Health Assessment and will resume assessment activities in the spring of 2022.  Our team will write a new Community Health Improvement Plan to capture the changing needs and concerns of the community. These endeavors are made possible through continued partnership and collaboration with our community partners.  

Please find below, a copy of our 2016 Community Health Assessment, our 2017 Community Health Improvement Plan, and our Progress Report for our 2017 Community Health Improvement Plan.
